Unable to verify SMSF information is an error from the ATO from which they have located a discrepancy between the data being used and the data held with the ATO for the SMSF.
The issue is likely one of the following:
provided ABN is not registered as an SMSF
SMSF ABN is not compliant or not regulated
SMSF FIA (financial institution account) and/or ESA is either incomplete or does not match ATO records

It would be best to check everything is up to date and that the ATO has all the correct information. To check the first 2 points, please search the SMSF ABN within the superfundlookup website (https://superfundlookup.gov.au/ ) and provided the regulatory details are valid, the issue is likely related to the third point - the ESA or bank account details need to be updated with the ATO.
Once updated, the rollover needs to be initiated again and provided the ATO doesn't fail the rollover attempt again due to a discrepancy in details, the rollover should hopefully go through on that attempt.
